Job Description:
Willows Care Center is seeking a dedicated and experienced Physical Therapist to join our team. The ideal candidate will provide high-quality physical therapy services to our patients, promoting wellness and enhancing their quality of life.
Responsibilities:
- Perform comprehensive assessments and develop individualized treatment plans for patients.
- Administer physical therapy interventions and modalities to improve mobility, strength, and function.
- Collaborate with healthcare professionals to coordinate patient care and achieve optimal outcomes.
- Educate patients and families on home exercise programs and self-management strategies.
- Maintain accurate and timely documentation of patient progress and treatment interventions.
Qualifications:
- Valid Physical Therapist license in the state of California.
- Minimum of 3 years of experience as a Physical Therapist, preferably in a long-term care or rehabilitation setting.
- Strong clinical skills and knowledge of evidence-based physical therapy practices.
- Excellent communication, interpersonal, and problem-solving abilities.
- Ability to work independently and as part of a multidisciplinary team.
